Request for Chemicals Requiring Approval

If you wish to order a chemical for use on the campus of UA, you should first check the list of Chemicals Requiring Prior Approval.  If your material is found on this list, you should complete the approval request below:

Please note:  If using BuyBama please disregard this approval process.  BuyBama utilizes a separate approval process. This is a two page form – the first page is for you, the second page is for EHS, but the ‘submit’ button is at the bottom of the second page.  Please be sure you scroll to the end and press ‘submit’.

Request Approval for Chemical Orders

  • If you are purchasing a replacement chemical and have already submitted an SOP you may provide the previous approval number in lieu of resubmitting an SOP. If you have never submitted an SOP for your chemical you will need to attach an SOP. If an SOP and/or prior approval number are not submitted the request will default in the system and not be sent for review.
  • EHS will review the chemical order and send an email with your approval or a request for more information within the next four (4) business hours. Approval numbers as well as shipping information will be included in this email.
