On-the-job injuries, illnesses, and near-misses must be reported to the Office of Risk Management as soon as is reasonably possible. Initial reports should be made to supervisors immediately.
All injuries, regardless of severity, must be reported to supervisors immediately. Following supervisor notification, the Injury and Illness Report should be completed, and followed up by visiting the Faculty/Staff clinic at University Medical Center for injuries occurring during normal business hours or the DCH Regional Medical Center Emergency Department for injuries occurring after hours.
Near-misses should also be reported through the Injury and Illness Report to help improve safety measures. When reporting a near-miss, select “I am not making an OJI claim. This is for reporting purposes only” on the Injury and Illness Report. Treatment at a medical facility is not required for near-misses.
If your injury is an emergency, go immediately to the nearest emergency department. As soon as is reasonably possible, report the injury to your supervisor, then complete the Injury and Illness Report and contact Risk Management.
